Role Overview

Work collaboratively within a dedicated team, assessing each resident's unique interests and differences. Your role focuses on fostering social, emotional, and developmental growth through tailored, person-centered care.

Key Responsibilities

• Assess residents' psychosocial needs and co-create individualized care plans.

• Organize and lead group or one-on-one activities to boost social, emotional, and developmental well-being.

• Champion residents' rights, independence, and dignity in all interactions.

• Offer emotional, practical, and bereavement support to residents and families.

• Track, document, and report on residents' progress and activity involvement.

• Keep precise, confidential records per organizational standards.

• Help sustain a safe, positive, respectful environment for everyone.

• Refer to external services and link families with vital support resources.
