Key Responsibilities:
- Deliver professional and person-centred care to clients with intellectual disabilities and complex care needs.
- Build positive relationships with clients, gaining their trust and respect.
- Assess, plan, implement, and evaluate social care programmes in line with professional standards.
- Work effectively both as part of a team and independently.
- Ensure the highest professional standards in patient care using an evidence-based, care-planning approach.
- Empower clients by promoting their strengths and supporting them in addressing their challenges.
- Collaborate with the multi-disciplinary healthcare team to ensure the best outcomes for clients.
- Provide practical support and assistance tailored to each client’s needs.
Required Education, Skills, and Qualifications:
- Degree in Social Care
- Experience with disabilities and behavioural issues
- Valid CORU registration
- Ability to work as part of a multi-disciplinary team.
- Previous experience is required
- Own car and a valid driving licence are preferred
